Self promotion

Self promotion was a very selfish act, someone who self promoted himself promoted himself to a higher rank without the approval of the person who should have decided if he deserved the promotion. Duh! Sith Lords liked to promote them selves, but most Sith who promoted themselves died when the planet they were on got blown up. One Sith that got away with promoting himself was Darth Elmo, who promoted himself to Sith Mega-Super-Overlord.
